Confidential Hospice Provider in Saint Charles, IL is seeking a compassionate Hospice RN Case Manager to join our field-based team delivering in-home end-of-life care. You will conduct assessments, develop care plans, and provide skilled nursing visits to ensure patient comfort.

Qualifications

• Active RN license in Illinois.
• 1–2 years clinical nursing experience; hospice/home health preferred.
• CPR certification.

Responsibilities

• Perform skilled nursing visits to monitor symptoms and comfort.
• Collaborate with physicians, social workers, chaplains, and aides.
