Job summary

Star Hospice, a part of LHC Group and in the Optum family, is hiring a Registered Nurse Case Manager to deliver comprehensive hospice care at patients' homes in Illinois. You will assess, plan, and coordinate the Plan of Care with an emphasis on physical, psychosocial, emotional, and spiritual well-being, collaborating with the care team.

This role requires an active RN license, reliable transportation, and at least 1 year of clinical experience.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license and access to a dependable vehicle or reliable transportation.
• 1+ years of clinical experience.

Responsibilities

• Makes the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identifies physical, psychosocial, and environmental needs and reassesses at least every 15 days.
• Initiates and coordinates the plan of care.
• Documents problems, goals, interventions, and patient/family response to care.
• Collaborates with patient/family, attending physician and IDG members in daily care.
• Instructs and supervises patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes on patient condition.
• Work with diverse patients regardless of background.
