Job summary

UnitedHealth Group is hiring a Registered Nurse Case Manager for Star Hospice in Illinois. You will oversee the Plan of Care for hospice patients, collaborating with a multidisciplinary team to promote comfort and quality of life for individuals at home.

You will perform initial assessments within 48 hours of referral, develop care plans, document progress, and coordinate with physicians and caregivers. Requires active RN license, valid driver's license, and 1+ year of clinical experience.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation.
• 1+ years of clinical experience.

Responsibilities

• Makes the initial nursing evaluation in determining eligibility for hospice services during visit within 48 hours of referral.
• Identifies physical, psychosocial, and environmental needs and re-assesses as needed.
• Initiates and coordinates the plan of care.
• Documents problems, goals, interventions, and patient/family response to hospice care.
• Collaborates with patient/family, attending physician and IDG members in daily care.
• Instructs and supervises patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes regarding patient condition.
• Function in diverse home settings regardless of age, race, creed, or disability.
