Job summary

Affinity Hospice is seeking an RN Case Manager to join the interdisciplinary hospice nursing team in Illinois. You will coordinate care for patients and families after identifying needs across physical, psychosocial, and spiritual domains, initiating appropriate interventions, and providing continuous, individualized care plans.

On-call visits, admissions, and death visits may be required. The role emphasizes coordination with the IDG, adherence to regulations, and comprehensive charting within

Qualifications

• Read, write, and speak English.
• Work independently and under the direction of the physician and management.
• Drive self to and from patient visits and the office.
• Experience with tablets and smart phones.

Responsibilities

• Coordinate care and assess patient and family needs from admission through hospice stay.
• Ensure compliance with hospice regulations and HIPAA.
• Educate patient, family and primary care givers on the disease process, self-care techniques, end-of-life care.
• Chart progress and complete charting within 24 hours of patient visits.
• Supervise and collaborate with LPN/LVN, CNAs, social workers, and chaplains to meet the plan of care.
• Participate in on-call needs, admissions and routine visits.
