Job summary

Laws & Associates is seeking a Hospice RN Case Manager in Illinois to coordinate care for patients in home-based settings, working with an interdisciplinary team to ensure comfort, dignity, and quality of life.

The role features a Monday–Friday schedule with limited on-call, dedicated after-hours RN support, and additional compensation for on-call coverage. Openings in several IL suburbs include Elmhurst, McHenry, Mount Carmel, Mount Pleasant, Mount Vernon, Northbrook, and Wheeling.

Qualifications

• 1+ year of hospice or home health experience.
• Experience in case management or care coordination.
• Strong clinical judgment and ability to work independently.
• Patient-centered, end-of-life care approach.

Responsibilities

• Managing patient care in hospice/home health settings.
• Coordinating with physicians and interdisciplinary care teams.
• Supporting patients and families through all stages of care.
• Providing leadership and guidance within the care team.
