Job summary

Chapters Health System in Florida Largo seeks an RN Case Manager to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Team, and provide clinical, palliative and supportive care to maintain the patient at home where possible.

The role requires an active Florida RN license, hospice or hospital experience, EMR/EHR familiarity, and a valid driver’s license with reliable transportation. BLS certification is also required.

Qualifications

• Active RN license in the state of employment.
• Minimum of 1 year nursing experience in hospice or hospital setting.
• Experience with EMR/EHR systems.
• Mobile driver with valid license and automobile insurance.
• Reliable transportation to meet visit schedules.
• Ability to communicate effectively in English (verbal and written).
• Ability to visit patients in their homes for assessments.
• Ability to perform essential functions and physical requirements, with or without accommodation.
• BLS for healthcare professionals (American Heart Association or Red Cross).

Responsibilities

• Provide direct care to patients and families as part of the Interdisciplinary Team.
• Coordinate Plan of Care reflecting patient’s evolving needs.
• Educate patients, families and caregivers about disease, palliative care and safety.
• Report changes to IDT and obtain physician orders as needed.
• Support referrals and coordinate with hospitals, physicians and facilities.
• Communicate clearly with physicians and staff; SBAR as needed.
• Supervise Home Health Aides and CHWs when requested.
