Job summary

Chapters Health System in Florida is seeking an RN Case Manager to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Team, and provide clinical, palliative and supportive care to keep participants in their home environment whenever possible.

The role requires an active Florida RN license, BLS, and communication skills; hospice or hospital experience is preferred; EMR/EHR experience, valid driver’s license, reliable transportation, and the ability to perform

Qualifications

• Active Florida RN license (required).
• Minimum 1 year nursing experience in hospice or hospital setting preferred.
• Experience with EMR/EHR systems.
• Valid driver’s license and automobile insurance.
• Reliable transportation to meet visit schedule.
• Ability to communicate effectively in English (verbal and written).
• Ability to visit participants in their homes for assessments.
• Ability to perform essential functions including lifting patients/equipment.

Responsibilities

• Provides and manages direct care to patients and families as part of the Interdisciplinary Team (IDT).
• Coordinates the Plan of Care to reflect evolving patient needs.
• Educates patient, family and caregivers about disease process, prevention and palliative care.
• Supports home visits and safety assessments to enable home-based care.
• Communicates changes in patient condition to IDT and other health professionals.
• Participates in IDT meetings and documents pertinent information.
• Supervises care delivery by Home Health Aides as requested.
• Maintains appropriate professional and safety standards at all times.
