Optimal Hospice is seeking a Registered Nurse Case Manager to deliver high-quality hospice care in Colorado. You will conduct comprehensive patient assessments, develop and revise care plans, administer medications, and coordinate care with the interdisciplinary team and family.

The role requires an active RN license, CPR certification, and strong communication skills. Hospice experience is preferred, along with a valid driver's license.

Qualifications

• Registered nurse with current licensure to practice in the state.
• Graduate of National League for Nursing accredited school of nursing.
• Maintains a current CPR certification.
• Current valid driver's license.

Responsabilités

• Completes initial, comprehensive and ongoing patient assessments and develops a plan of care.
• Provides professional nursing care following the nursing process.
• Assesses and reevaluates patient status and adjusts the plan of care as needed.
• Initiates nursing actions, administers prescribed medications and treatments.
• Counsels patient and family on nursing and related needs and provides health instructions.
• Acts as Case Manager and coordinates patient care for assigned caseload.
• Maintains accurate clinical notes and records changes in pain/symptom management.
• Communicates with physicians and community health professionals to coordinate care.
• Works with interdisciplinary team to meet emotional needs of patient and family.
