Job summary

Optimal Hospice is seeking a Registered Nurse Case Manager to provide comprehensive hospice care for patients in Parker, Colorado. The role focuses on patient assessments, care planning, medication administration, and coordination with the interdisciplinary team.

The nurse must have active RN licensure, CPR certification, and a clean driving record; hospice experience is preferred. You will contribute to compassionate, patient-centered care within a supportive team environment.

Qualifications

• Must hold current RN licensure in the state.
• Graduate of NLN-accredited nursing program.
• Current CPR certification required.
• Valid driver’s license required.
• At least 1 year Hospice experience preferred.

Responsibilities

• Completes initial and ongoing patient/family assessments to determine hospice needs.
• Develops and revises care plans based on nursing diagnosis and patient goals.
• Administers medications and treatments per physician orders.
• Acts as Case Manager coordinating patient care across the interdisciplinary team.
• Maintains accurate clinical notes and documents changes in patient condition.
• Communicates with physicians and community health professionals to coordinate care.
• Educates patient and family on self-care and disease management.
• Ensures a safe environment and supports patient independence and comfort.
