Job summary

Optimal Hospice is seeking a Registered Nurse Case Manager to deliver comprehensive nursing services to our patients. You will perform initial and ongoing assessments, develop care plans, and coordinate patient care with the interdisciplinary team.

This role requires current RN licensure, NLN-accredited nursing program, CPR certification, and a valid driver's license. Hospice experience is preferred, with opportunities for professional growth within a supportive team.

Qualifications

• Current RN licensure to practice in the state.
• Graduate of NLN-accredited school of nursing.
• CPR certification current.
• Current valid driver's license.
• Hospice experience preferred (1 year).
• Excellent observation, problem solving, and communication skills.
• Ability to organize and prioritize workload independently.

Responsibilities

• Completes initial, comprehensive and ongoing assessments of patients and families to determine hospice needs.
• Provides professional nursing care by utilizing all elements of the nursing process.
• Assesses and evaluates patient's status and updates the plan of care as needed.
• Develops a care plan with goals based on nursing diagnosis and includes patient/family.
• Administers medications and treatments as prescribed by the physician.
• Counsels patient and family in nursing and related needs and self-care.
• Provides and maintains a safe environment for patients and continuity of care.
• Acts as Case Manager when assigned and coordinates patient care with the Clinical Supervisor.
• Maintains accurate clinical notes and documents pain/symptom changes.
• Communicates with physicians and community health professionals to coordinate care.
• Attends interdisciplinary group meetings.
