Affinity Hospice seeks a Registered Nurse/Case Manager to plan, direct, and provide hospice care in home settings. You will build on community resources to serve patients and families, coordinating with interdisciplinary teams to meet individual needs.

The role requires RN licensure, CPR, and at least two years of relevant experience, with on-call responsibilities and supervision of aides and LPNs. A driving requirement and strong communication are essential.

Competenze

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State license as a Registered Nurse (RN).
• Current CPR certification.
• Minimum of two years of RN experience in clinical care, home health, or hospice preferred.
• Must be a licensed driver with insured vehicle in good working order.
• Strong observation, communication, problem solving, and basic math skills.

Mansioni

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Implement, develop, and document the patient plan of care.
• Provide care using infection control measures to protect staff and patients (OSHA).
• Ensure continuity of quality patient care with proper documentation.
• Monitor cases to ensure third-party payer requirements are met.
• Demonstrate commitment, professional growth, and competency.
• Promote agency philosophy and administrative policies.
• Perform on-call responsibilities and provide on-call service as assigned.
• Supervise hospice aides and licensed practical nurses.
