Job summary
Optimal Hospice is seeking a Registered Nurse Case Manager to deliver comprehensive nursing care and coordinated plan of care for hospice patients in the Parker, CO area. You will assess patients, develop and adjust care plans, administer medications, educate families, and collaborate with the interdisciplinary team to ensure comfort and quality of life.
The ideal candidate has an active RN license, CPR certification, and at least one year of hospice experience.
Qualifications
- Must hold an active RN license in the state.
- Graduate of an NLN-accredited school of nursing.
- Maintains current CPR certification.
- Current valid driver’s license.
Responsibilities
- Performs comprehensive patient and family assessments to determine hospice needs.
- Provides professional nursing care using the nursing process.
- Develops and updates plans of care with patient and family involvement.
- Administers medications and treatments as prescribed.
- Coordinates patient care as Case Manager with the clinical team.
- Maintains accurate and timely clinical notes and reports changes in patient condition.
- Educates patients and families on self-care and disease management.
- Collaborates with interdisciplinary team to meet emotional and care needs.
- Attends interdisciplinary group meetings.
