Job summary

Compassus in Colorado is seeking a licensed RN to join our Hospice RN Case Manager team, providing compassionate at-home care and coordinating patient plans with the care team.

You'll empower families, educate caregivers, and help patients spend meaningful time with loved ones while adhering to best-practice hospice standards. A driver’s license and CPR/BCLS are required with hospice experience preferred.

Qualifications

• Licensed RN in the state you work
• Minimum 2 years of nursing experience, hospice experience preferred
• Current driver’s license
• CPR/BCLS certification currently valid

Responsibilities

• Provide compassionate nursing care to hospice patients in home settings
• Coordinate plans of care with the interdisciplinary care team
• Educate and empower caregivers on care for loved ones
• Document patient status and changes in medical records
• Collaborate with the care team to ensure comfort and dignity for patients
