Job summary

Commonwealth Hospice, part of the LHC Group family and UnitedHealth Group, is seeking a Registered Nurse Case Manager for home visits. You will deliver the Plan of Care for hospice patients, collaborate with professionals, and promote physical, psychosocial, emotional, and spiritual well-being.

You will work full-time with on-call rotation evenings/weekends. VA RN licensure and reliable transportation are required; the agency covers a 50-mile radius for patient visits.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Make the initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identify and reassess physical, psychosocial, and environmental needs for ongoing care.
• Initiate and coordinate the plan of care with the patient/family and IDG members.
