Job summary

Commonwealth Hospice, part of the Optum family, seeks an RN Case Manager to deliver the Plan of Care for hospice patients through home visits. VA RN licensure is required; include collaboration with the IDG and other professionals to promote physical, psychosocial, and spiritual well-being.

Qualifications

• Current and unrestricted RN licensure in VA
• Current driver's license and access to a dependable vehicle or public transportation
• VA: Experience with terminally ill required
• 1+ years of clinical experience
• Current CPR certification or ability to complete within 90 days of hire

Responsibilities

• Makes the initial nursing evaluation within 48 hours of referral.
• Identifies physical, psychosocial, and environmental needs and re-assesses at least every 15 days.
• Initiates and coordinates the plan of care.
• Documents problems, goals, interventions, and patient/family response to hospice care.
• Collaborates with patient/family, attending physician and IDG members in daily care.
• Instructs and supervises patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes regarding the patient's condition.
