Job summary

UnitedHealth Group seeks a Registered Nurse Case Manager for hospice home-visit care. You will assess, plan, implement, and document the Plan of Care, collaborating with families and the IDG to support physical, psychosocial, emotional, and spiritual well-being.

OH RN license required; valid driver’s license and dependable vehicle or public transport. On-call evenings/weekends and a 50-mile service radius apply. Competitive salary and comprehensive benefits offered.

Qualifications

• Current and unrestricted RN licensure in the state of practice (OH).
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• CPR certification current or ability to obtain within 90 days of hire.
• 1+ years of clinical experience (preferred).

Responsibilities

• Makes the initial nursing evaluation within 48 hours of referral.
• Identifies physical, psychosocial, and environmental needs and reassesses every 15 days.
• Initiates and coordinates the plan of care; documents and tracks patient response.
• Collaborates with patient/family, attending physician and IDG members daily.
• Instructs and supervises patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes and can work in diverse home settings.
