Job summary

Commonwealth Hospice, part of LHC Group and UnitedHealth Group family, is seeking a Registered Nurse Case Manager to deliver and coordinate the Plan of Care for hospice patients in home settings.

You will perform initial nursing evaluations within 48 hours of referral, develop the care plan with the care team, and ensure quality, compassionate care within a 50 mile radius of your assigned area. VA RN license and reliable transportation required.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.

Responsibilities

• Makes the initial nursing evaluation in determining eligibility for hospice services within 48 hours of referral.
• Identifies patient/family physical, psychosocial, and environmental needs and re-assesses at least every 15 days.
• Initiates and coordinates the plan of care with the IDG.
• Documents problems, goals, interventions, and patient/family response to hospice care.
• Collaborates with patient/family, attending physician and other IDG members in daily care.
