Job summary

UnitedHealth Group (Access Hospice Care, a part of LHC Group) invites you to join a nationwide post-acute care partner dedicated to compassionate, high-quality patient care. As the Registered Nurse Case Manager, you will lead the Plan of Care for hospice patients, collaborate with colleagues, and promote physical, psychosocial, emotional, and spiritual well-being.

We are hiring for Full-Time and PRN; compensation includes a salary range and comprehensive benefits.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current CPR certification or ability to obtain within 90 days of hire.
• Current driver’s license and access to reliable transportation.

Responsibilities

• Makes the initial nursing evaluation for hospice eligibility within 48 hours of referral.
• Identify patient/family needs and reassess at least every 15 days.
• Initiate and coordinate the plan of care with the IDG.
