Job summary

Lifecare Home Health is seeking an RN Case Manager to perform admissions and coordinate nursing care for patients in their homes. You will work with the interdisciplinary team and referring agencies to organize comprehensive care plans.

Responsibilities include conducting patient assessments, documenting plans of care, and ensuring timely paperwork. Strong communication with physicians and families is essential, with on-call responsibilities as assigned.

Qualifications

• Two years experience as a Registered Nurse in a clinical care setting.
• One year full-time hospice experience preferred.
• Current State license as a Registered Nurse; valid driver’s license.

Responsibilities

• Assess hospice patients using nursing assessment and LCD criteria to identify needs.
• Develop and document the plan of care in alignment with assessments.
• Provide nursing care with infection control measures (OSHA).
• Collaborate with Interdisciplinary Group/Team to meet patient/family needs.
• Communicate with attending and hospice physicians involved in care.
