Job summary

Capital Caring Health in Virginia is seeking an RN, Case Manager to assess patient/family needs, coordinate the Plan of Care with the IDT, and provide clinical, palliative and supportive care to keep the patient in their home as long as possible.

The role requires an active RN license, at least 1 year of nursing experience (hospice or hospital preferred), EMR/EHR experience, and a valid driver’s license with reliable transportation to meet visit schedules.

Qualifications

• Current license as RN in the state where the employee will be working.
• Minimum of one year nursing experience; hospice or hospital experience preferred.
• Experience with EMR/EHR systems.
• Mobile driver with valid license and insurance per policy.
• Reliable transportation to meet visit schedules.
• Active BLS certification for healthcare professionals.

Responsibilities

• Provides and manages direct care to patients and families as part of IDT.
• Coordinates the Plan of Care reflecting evolving patient needs.
• Educates patients and families about disease, palliation, and safety.
• Home visits to assess safety, medication and nutritional needs.
• Communicates with physicians and IDT; documents changes timely.
• Develops relationships with facilities and care teams.
