Job summary

Chapters Health System in Virginia seeks an RN Case Manager to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Team (IDT), and provide clinical, palliative and supportive care to maintain the patient at home where possible.

RN license in the state, 1 year hospice or hospital experience preferred, and ability to travel to patient homes are required. Strong communication and documentation are essential.

Qualifications

• Current RN license in the state of employment.
• 1 year nursing hospice or hospital experience preferred.
• Alzheimer's/Dementias training may be required in some locations.
• EMR/EHR experience is preferred.
• Valid driver’s license and reliable transportation.
• BLS for healthcare professionals (American Heart Association or Red Cross).
• Ability to communicate clearly in English (verbal and written).
• Capable of home visits and physical requirements of the job.

Responsibilities

• Provides and manages direct care to patients and families as part of IDT.
• Coordinates the Plan of Care reflecting evolving patient needs.
• Educates patients and families about disease, palliation and safety.
• Home visits to assess safety, medication and nutrition compliance.
• Communicates changes to IDT and initiates physician orders.
• Represents the company at facilities and coordinates referrals.
