Job summary

Affinity Hospice Management LLC is seeking a dedicated Registered Nurse to plan, organize, and direct hospice care delivered in patients' homes and communities. The role emphasizes community health education and collaboration with families to meet patient needs.

You will perform comprehensive assessments, develop and adjust care plans, administer medications, counsel families, and coordinate with the interdisciplinary team.

Qualifications

• Graduate of National League of Nursing accredited school of nursing.
• Current licensure in State.
• RN with 1 year of experience within the last 3 years, or a BSN with RN license; 2 years preferred in public health, home care, or hospice is helpful.

Responsibilities

• Complete initial and ongoing comprehensive assessment of patient and family to determine hospice needs.
• Provide professional nursing care by utilizing all elements of the nursing process.
• Develop and initiate plan of care and revise as patient status changes.
• Administer medications and treatments as prescribed by the physician.
• Counsel patient and family in meeting nursing and related needs and provide health instructions.
• Act as Case Manager for assigned caseload when directed by Clinical Supervisor.
• Collaborate with interdisciplinary team and participate in quality assessment performance improvement activities.
