Job summary

Gentiva is seeking a dedicated Hospice RN / RN Case Manager to deliver compassionate care in patients' homes across Indianapolis, IN. You will assess, plan, and coordinate skilled nursing services with an interdisciplinary team to support patients and families facing life-limiting illness.

Ideal candidates hold an Indiana RN license with at least 1 year of hospice or home health experience. CPR certification is required, and you will work in a patient-centered, supportive environment that

Qualifications

Active RN license in Indiana.
Minimum 1 year RN experience in hospice, home health, or related settings.
Hospice or palliative care experience preferred.
CPR certification required.

Responsibilities

Perform comprehensive RN assessments and ongoing evaluations, in person or telephonically.
Deliver and document skilled nursing care per Plan of Care in patient homes or facilities.
Coordinate with physicians, LPNs, CNAs, social workers, and chaplains on care plans.
Educate patients and families on disease process, end-of-life care, and self-care strategies.
Maintain timely clinical documentation and ensure infection control compliance.
