Job summary

Atrium Health is seeking a Hospice Nurse Case Manager (RN) to coordinate end-of-life care for patients and families in home hospice in Floyd County. The role leads the IDG, develops the care plan, and ensures regulatory compliance.

Responsibilities include patient/family education, EHR documentation, medication administration, discharge planning, and supervision of home health staff. Minimum 1–2 years in related nursing required.

Qualifications

• Education: Accredited nursing program completion.
• 1–2 years nursing experience in home health, hospice, or related areas.
• RN licensure required and active.

Responsibilities

• Coordinate patient/family care in home hospice settings to maximize quality of life.
• Educate patients and families about disease trajectory, hospice principles, and care planning.
• Lead the IDG, develop and revise the plan of care, and coordinate services with the team.
• Administer medications and treatments as prescribed and document in the EHR.
• Oversee discharge planning and supervise Home Health Aide and LPN staff.
