Job summary

Atrium Health Floyd is seeking a Hospice Nurse Case Manager (RN) to coordinate comprehensive home hospice care across Floyd area. The role includes assessments, care planning, and direct nursing in patients' homes, with collaboration from an interdisciplinary team.

You will educate patients and families, ensure compliance with hospice standards, and support care coordination and documentation. Full-time, M–F 8:00 AM–5:00 PM with occasional on-call offers meaningful, patient-centered care.

Qualifications

• Graduate of an accredited Registered Nursing program.
• Proven bedside nursing experience in hospice or end-of-life care preferred.
• Strong assessment, communication, and critical-thinking skills.

Responsibilities

• Conduct admission, routine, urgent, and pronouncement visits.
• Develop and revise individualized patient care plans.
• Provide skilled nursing care and symptom management in home hospice.
• Maintain accurate clinical documentation and collaborate with the care team.
