Job summary

Affinity Hospice in Georgia is seeking an experienced registered nurse to join our care team as a Case Manager. You will plan, direct and provide hospice care in home settings, coordinating with families and an interdisciplinary team. This full-time role requires current RN license, CPR, and a minimum of two years of hospice/clinical experience.

Qualifications

• Graduate of an accredited nursing program (Diploma/ADN/BSN).
• Current Georgia RN license.
• Current CPR certification.
• Minimum two years RN experience in clinical/home health/hospice preferred.
• Ability to lift and move patients and equipment.
• Excellent observation and communication skills.
• Valid driver's license with insured vehicle.
• Nursing skills per competency checklist.

Responsibilities

• Provide clinical direction to interdisciplinary team and family.
• Implement and document the patient plan of care.
• Ensure infection control measures for staff and patient.
• Maintain documentation for quality and payer requirements.
• Supervise hospice aides and licensed practical nurses.
• Perform on-call duties as assigned.
• Support department goals and initiatives.
