Job summary

Atrium Health Floyd is seeking a Hospice Nurse Case Manager (RN) to coordinate care for terminally ill patients and families across hospice settings in Floyd County. You will lead the IDG, develop and update the Plan of Care, and ensure timely documentation and communication with physicians.

The role requires RN licensure, BLS, 1-2 years of nursing experience (hospice/home health preferred), and strong assessment, teaching and teamwork skills.

Qualifications

• Completion of an accredited nursing program and RN licensure.
• BLS for Healthcare Providers certification required upon hire.
• 1–2 years of nursing experience, preferably in home health, hospice, or related fields.

Responsibilities

• Identify and coordinate patient/family care to support terminally ill patients across hospice settings.
• Lead the interdisciplinary care group and develop the patient’s Plan of Care.
• Document in the electronic health record and communicate changes to physicians and IDG.
• Provide ongoing assessment of patient response to treatments and adjust care plans as needed.
• Supervise home health staff visits and participate in discharge planning.
