Job summary

Atrium Health is seeking a Hospice Nurse Case Manager (RN) to coordinate comprehensive home-based hospice care across assigned service areas. The role involves direct patient visits, collaborative planning with the interdisciplinary team, and ongoing education for patients and families.

The position requires strong assessment and communication skills, GA RN licensure, and the ability to travel between patient locations in the service area. Full-time schedule with on-call rotation as needed.

Qualifications

• Graduate of an accredited Registered Nursing program.
• Current Georgia RN license and valid driver’s license required.
• Reliable transportation and ability to travel throughout the service area (home visits).

Responsibilities

• Provide skilled nursing care and symptom management in patients' homes.
• Conduct admissions, assessments, and develop individualized care plans.
• Coordinate with interdisciplinary team and document care appropriately.
• Educate patients and families on disease progression and end-of-life care.
• Maintain clinical documentation and ensure regulatory compliance.
