Job summary

Atrium Health Floyd is seeking a Hospice Nurse Case Manager (RN) to coordinate comprehensive home hospice care across Floyd area. The role includes assessments, care planning, and direct nursing in patients' homes, with collaboration from an interdisciplinary team.

Qualifications

As a Hospice Nurse Case Manager, you will provide skilled nursing care, coordinate interdisciplinary services, and support patients and caregivers through every stage of the hospice journey.

Position Summary

The Hospice Nurse Case Manager is responsible for managing and coordinating comprehensive hospice care for assigned patients. This role includes conducting assessments, developing individualized care plans, providing direct patient care in the home, collaborating with the interdisciplinary team, and serving as a clinical resource to patients, families, and caregivers.

Key Responsibilities
  • Conduct admission, pre-admission, routine, urgent, and pronouncement visits.
  • Perform comprehensive physical, psychosocial, and environmental assessments.
  • Develop, implement, and revise individualized patient care plans.
  • Provide skilled nursing care and symptom management in accordance with hospice standards.
  • Respond to patient needs, medical crises, and family concerns in a timely manner.
  • Supervise home health aides and collaborate with caregivers to ensure quality patient care.
  • Maintain accurate and timely clinical documentation.
Patient & Family Education
  • Educate patients, families, and caregivers on disease progression, symptom management, medications, and end-of-life care.
  • Teach caregivers how to safely perform tasks essential to patient care.
  • Provide emotional support and guidance to patients and families throughout the hospice experience.
Interdisciplinary Collaboration
  • Collaborate closely with physicians, social workers, chaplains, aides, and other members of the care team.
  • Communicate changes in patient condition and coordinate appropriate interventions.
  • Participate in interdisciplinary team meetings and care planning discussions.
Quality & Compliance
  • Adhere to hospice, state, federal, and regulatory requirements.
  • Follow infection prevention and control practices.
  • Participate in quality improvement and performance initiatives.
  • Ensure compliance with documentation and clinical practice standards.
Qualifications
Required Education
  • Graduate of an accredited Registered Nursing program
Required Licensure
  • Current Georgia Registered Nurse (RN) license
  • Valid Georgia driver's license
  • Reliable transportation and ability to travel throughout the service area
Required Experience
  • Minimum of two years of nursing experience
  • Experience providing compassionate care to patients and families experiencing serious illness or end-of-life care preferred
  • Strong assessment, communication, and critical-thinking skills
Physical Requirements
  • Ability to stand, walk, bend, lift, and assist patients as needed.
  • Ability to lift and carry up to 50 pounds occasionally.
  • Ability to work in a home care environment and travel between patient locations.
Schedule
  • Full-time, Monday through Friday, 8:00 AM – 5:00 PM
  • Occasional on-call responsibilities based on team rotation

The Hospice Case Manager coordinates care for a defined patient population including adolescent, adult, and geriatric terminally ill patients. This includes utilization management, transitional care planning, psychosocial and functional status assessment, patient advocacy, education for the patient/family, and monitoring value indicators to demonstrate outcomes resulting from the service provided. This individual collaborates with other case managers, other health care disciplines, patient financial services, and patient reimbursement plans to provide a comprehensive assessment of a given patient’s plan of care, goal/outcome attainment, and continued care needs. He/she provides care that is in accordance with hospice interdisciplinary plan of care for each patient and consistent with policies and procedures.
