Job summary

Patriot Healthcare is seeking a dedicated Registered Nurse Case Manager to coordinate and direct hospice services per physician orders and patient plans. You will work with an interdisciplinary team to deliver compassionate, compliant nursing care in South Carolina.

The role requires SC RN licensure and a nursing degree; hospice experience is preferred and a minimum of one year in hospital or long-term care settings is desirable.Competitive pay and comprehensive benefits are offered.

Qualifications

• Nursing degree from an accredited college/university.
• Licensed RN in the State of South Carolina.
• Hospice experience preferred.
• 1 year hospital/long-term care experience preferred.

Responsibilities

• Coordinate and direct provision of hospice services per physician orders and patient plan.
• Participate as a member of the Interdisciplinary Team (IDT).
• Follow hospice policies, state and federal regulations.
• Document direct patient care for accurate billing and records.
• Perform administrative duties and supervise Hospice Aides and CNA schedules.
