Job summary

Patriot Healthcare is seeking an RN Case Manager to provide and coordinate hospice services with patient-centered care in South Carolina. You will work with an interdisciplinary team to ensure compassionate, quality end-of-life care and support for patients and families.

The role requires SC RN licensure and a nursing degree, with preferred experience in hospital or long-term care and hospice settings. This is a full-time, on-site position in Anderson, SC.

Qualifications

• Must hold a Nursing Degree and SC RN license.
• 1 year of hospital or long-term care experience preferred.
• Hospice experience is a plus.

Responsibilities

• Coordinate hospice care per physician orders and patient plan.
• Serve as a member of the Interdisciplinary Team (IDT).
• Document accurately to support billing and quality initiatives.
• Provide education to caregivers and family members as needed.
• Maintain patient records and communicate with physicians and facility staff.
