Job summary

Patriot Healthcare in Columbia, SC is seeking an experienced RN Case Manager to coordinate hospice services with an interdisciplinary team. You will apply nursing processes to ensure comfort, dignity, and quality of life for patients and families.

You will work within hospice policies, document care accurately, and collaborate with physicians, facilities, and staff to optimize patient outcomes. A nursing license in South Carolina and 1 year of relevant experience are preferred.

Qualifications

• Must possess a Nursing Degree from an accredited college or university and be licensed as a Registered Nurse in the State of South Carolina.
• 1 year of experience as a nurse in a hospital, long-term care facility, or other related health care facility is preferred.
• Hospice experience also preferred.

Responsibilities

• Provide, coordinate, and direct hospice services according to physician orders and patient plan of treatment.
• Attend Interdisciplinary Team (IDT) meetings and follow hospice policies and nursing standards.
• Complete documentation and reporting to the Director of Clinical Services.
• Supervise Hospice Aide care and CNA schedules as needed.
• Maintain up-to-date patient records and coordinate care with physicians and facility staff.
