Job summary

Patriot Healthcare in South Carolina is seeking an experienced Registered Nurse Case Manager to coordinate hospice services according to physician orders and the patient plan of treatment.

You will work within the Interdisciplinary Team to deliver compassionate, compliant care, document nursing services, supervise Hospice Aide care and CNA schedules, and liaise with families to support quality of life.

Qualifications

• Must possess a Nursing Degree and be licensed as a Registered Nurse in South Carolina.
• At least 1 year of nursing experience in hospitals, long-term care facilities, or related health care facilities preferred.
• Hospice experience also preferred.

Responsibilities

• Coordinate and direct provision of hospice services per physician orders.
• Work with the Interdisciplinary Team to ensure quality of life for patients.
• Document nursing care and support billing as required.
• Supervise Hospice Aide care and CNA schedules.
• Report occupational exposures per hospice policies.
