Job summary

Patriot Healthcare in South Carolina is seeking an experienced Registered Nurse Case Manager to coordinate and direct hospice services under physician orders, ensuring dignity and comfort for patients and families. This role works within an interdisciplinary team, adheres to hospice policies and regulations, and reports to the Director of Clinical Services.

A SC RN license and nursing degree are required, with hospice experience preferred.

Qualifications

• Must have Nursing Degree and active SC RN license.
• Minimum 1 year nursing experience in hospital or long-term care.
• Hospice experience preferred.

Responsibilities

• Coordinate and direct hospice services per physician orders and patient plan.
• Serve as a member of the Interdisciplinary Team (IDT) and attend meetings.
• Follow hospice policies and regulatory requirements.
• Provide documentation to support professional billing of nursing services.
• Supervise Hospice Aide care and CNA schedules.
• Report exposures per hospice policies and procedures.
• Maintain up-to-date patient records with plans and changes.
• Coordinate communication with physicians and facility staff.
