Job summary

Patriot Healthcare is seeking a dedicated Registered Nurse Case Manager to coordinate and deliver hospice services with compassion. The role involves collaborating with the interdisciplinary team to ensure patient comfort, dignity, and timely documentation.

The RN will work closely with physicians, facility staff, patients, and families, maintaining records and supporting care plans in Anderson, South Carolina. A nursing degree and SC RN license are required, with hospice experience preferred.

Qualifications

• Must be a licensed Registered Nurse in South Carolina.
• 1 year of nursing experience in hospital, long-term care, or related health care facility preferred.
• Hospice experience also preferred.

Responsibilities

• Provide, coordinate, and direct hospice services per physician orders and patient plan of treatment.
• Serve as member of Interdisciplinary Team (IDT) and attend meetings.
• Document nursing care per hospice policies and guidelines.
• Report to Director of Clinical Services as needed for operations of nursing service.
• Support quality assurance programs and coordinate care with facility staff.
• Supervise Hospice Aide care and CNA schedules.
• Maintain patient records and communicate with physicians and families.
