Job summary

Patriot Healthcare is seeking an exceptional RN Case Manager in South Carolina to coordinate and direct hospice services per physician orders and patient plans. You will work with the interdisciplinary team to deliver compassionate, high-quality care and to support patients and families during life’s most challenging moments.

The role requires SC RN licensure and a nursing degree, with preference for hospice experience.

Qualifications

• Must possess a Nursing Degree from an accredited college or university.
• Licensed as a Registered Nurse in the State of South Carolina.
• 1 year of experience as a nurse in a hospital, long-term care facility, or other related health care facility preferred. Hospice experience also preferred.

Responsibilities

• Coordinate and direct hospice services according to physician orders and patient plan of treatment.
• Participate as a member of the Interdisciplinary Team (IDT) and attend meetings.
• Document nursing care and support compliant with hospice policies and regulations.
• Supervise Hospice Aide care and CNA schedules as needed.
