Job summary

Patriot Healthcare in South Carolina is seeking exceptional RN Case Managers to join a compassionate hospice team. The role centers on coordinating, directing, and delivering hospice services with a patient‑focused approach and adherence to physician orders and plans of care.

The RN will work within an Interdisciplinary Team, maintain documentation for quality and billing, supervise aides, and participate in care planning while upholding state hospice regulations and nursing standards.

Qualifications

• Must hold a nursing degree and SC RN license.
• 1 year hospital or long‑term care experience preferred.
• Hospice experience preferred.

Responsibilities

• Coordinate and direct hospice services per physician orders and patient plan.
• Serve as a member of the Interdisciplinary Team (IDT) and attend meetings.
• Ensure nursing standards and hospice policies are followed.
• Document direct patient care for billing and care planning.
• Supervise Hospice Aide care and CNA schedules.
