Job summary

Compassus is seeking a Hospice Registered Nurse / RN Case Manager to provide compassionate, patient-centered care at home or in hospice settings. You’ll coordinate plans, educate families, and collaborate with a multidisciplinary team to maintain comfort and dignity for patients and their loved ones.

The role requires an active RN licensure in the state of practice, CPR/BCLS certification, and prior hospice or home health experience. A driver’s license is also needed to visit patients.

Qualifications

• RN license in the state you work.
• Two years of experience as a registered nurse, hospice experience preferred.
• Current driver’s license.
• Current CPR and BCLS certification.
