Job summary

Compassus is seeking a Hospice Registered Nurse / RN Case Manager to provide compassionate, patient-centered hospice care. You will work with patients and families to support comfort, dignity, and quality of life in the home or facility setting.

Required are an active RN license in the state of employment, 2 years of RN experience (hospice preferred), current CPR/BCLS, and a valid driver’s license. Competitive pay, benefits, and onboarding are included as part of a supportive, family-focused

Qualifications

RN license in the state you work
Two years of experience as a registered nurse, hospice experience preferred
Current driver’s license
Current CPR and BCLS certification

Responsibilities

Deliver compassionate, patient-centered hospice care in collaboration with our care team
Build trusting relationships with patients and families to support quality of life
Educate caregivers on care needs and at-home management
Coordinate with the interdisciplinary team to provide comfort, peace and dignity at end of life
