Job summary

Community Health Network, Inc. seeks an experienced Hospice RN Case Manager to care for patients in facilities across the Lexington/Acton/Concord/Watertown area in Massachusetts. This full-time role features a Monday–Friday schedule with no weekends or on-call required.

You will provide direct patient care, coordinate with families and the care team, and advocate for quality end-of-life care. Mileage reimbursement is offered, along with comprehensive benefits and growth opportunities.

Qualifications

• Minimum 1 year of Hospice RN experience with a Medicare-certified agency.
• Direct patient care, coordination, and advocacy for Hospice patients.
• Comfortable with end-of-life care, death and dying.

Responsibilities

• Provide direct patient care and coordinate care for Hospice patients in the Lexington/Acton/Concord/Watertown area.
• Collaborate with families and the care team to support end-of-life planning.
• Schedule and manage visits; typical 40-hour Monday–Friday work week.
• Mileage reimbursement provided.

Well established hospice agency has a need for an experienced Hospice RN Case Manager to see patients in Facilities in the Lexington/Acton/Concord/Watertown, MA area and surrounding towns.




Requires minimum 1 year of Hospice RN experience with a Medicare certified agency.




Primary nurse provides quality nursing care to identified Hospice patients. The scope of the primary Hospice RN includes direct patient care, coordination of patient and patient advocacy. Patient includes the individual, family/significant other and community. Must be comfortable with issues relative to end-of life care, death and dying.




Full-time, Monday-Friday, 40 hours/week

Shorter works weeks considered.

No weekends or on-call required!
