Job summary

PruittHealth Hospice seeks a Registered Nurse Case Manager (RN) to provide compassionate hospice care in patients\' homes or care facilities. This on-call weeknight role (Mon-Thurs 5pm-8am) features flexible shifts, competitive pay, and strong benefits.

You will complete assessments, coordinate care with families, and document in the EMR while upholding policy and quality standards. Active GA RN license required; on-call experience preferred.

Qualifications

• Current, active RN license in the state of practice (GA).
• Hospice or home health experience preferred.
• Strong communication and documentation skills.
• Ability to work on-call schedule.

Responsibilities

• Complete evaluations, assessments, and treatments per policy and care plan.
• Complete all documentation promptly and accurately.
• Follow established policies and scope of practice.
• Maintain continuity of patient care in a hospice setting.
