PruittHealth Hospice is seeking a Registered Nurse Case Manager (RN) for on-call weeknights to deliver compassionate hospice and home health care across Georgia. You will assess patients, develop care plans, and coordinate services in partnership with families and a multidisciplinary team.

This role emphasizes accurate documentation, adherence to policies, and continuity of care in the patient's home or facility.

Qualifications

• Current, active RN license in Georgia.
• Hospice or home health care experience preferred.
• Ability to complete evaluations and develop care plans.
• Maintain accurate and timely documentation.

Responsabilités

• Complete evaluations, assessments, and treatments in accordance with policy and the individualized care plan.
• Complete all documentation and records in a timely and accurate manner.
• Follow established policies and procedures as related to the scope of practice.
• Maintain continuity of patient care in a hospice setting.
