Job summary

Eternal Faith Hospice Care seeks an RN Case Manager to perform admissions and provide nursing care for patients in their homes. You will coordinate care with the Interdisciplinary Group/Team, patient and family, and referring agency, ensuring timely paperwork and adherence to hospice guidelines.

The role requires 2 years of RN clinical experience with hospice preferred, GA licensure, and reliable transportation. Schedule is M-Th 5P-8A, with on-call expectations and local travel to patient homes.

Qualifications

• Two years nursing experience in a clinical setting with one year hospice preferred.
• Current State RN license; valid driver's license.

Responsibilities

• Assess hospice patients and identify physical, psycho-social, and environmental needs.
• Develop and document the plan of care aligned with assessments.
• Provide nursing care maintaining infection control standards.
