Job summary

Gentiva Hospice seeks a dedicated Registered Nurse Case Manager to provide in-home hospice and palliative care. You will assess patients, develop and update plans of care, and coordinate with an interdisciplinary team to ensure compassionate, quality outcomes.

You will educate families, manage symptoms, document care, and participate in on-call coverage as needed. Valid RN license in New Jersey and CPR are required; hospice experience preferred.

Qualifications

• Registered Nurse licensed in the state of employment.
• Minimum of one year experience as an RN in hospice, home health, or related settings.

Responsibilities

• Perform comprehensive patient assessments and educate patients, family members, and caregivers.
• Deliver skilled nursing interventions and symptom management per care plan.
• Develop and update individualized plans of care including visit frequency and risk identification.
• Coordinate care across interdisciplinary teams including physicians and nurse practitioners.
• Provide patient and caregiver education on disease, self-care, end-of-life care, medications, nutrition, and dietary needs.
• Document timely clinical records and ensure infection control and regulatory compliance.
