Job summary

Suncrest Hospice in Parsippany-Troy Hills, NJ, seeks an RN Case Manager to provide in-home and facility visits, delivering compassionate clinical care. The role works Monday–Friday daytime hours as part of an interdisciplinary team, with travel across the service area.

Candidates must hold an active RN license in good standing in NJ, excel in coordination with nurses, social workers, chaplains, and CNA leadership, and be proficient with electronic medical records.

Qualifications

• Current RN license in good standing in NJ.
• Ability to collaborate with an interdisciplinary team.
• Willingness to travel across the service area with reliable transportation.
• Experience with hospice rules and regulations and EMR systems.

Responsibilities

• Provide care and services for a group of patients, visiting patients where they reside (homes, SNFs, assisted living).
• Serve as part of the interdisciplinary team, contributing psych-social assessments and interventions.
• Coordinate with nurses, social workers, chaplains, volunteers, and CNA leadership.
