Gentiva Hospice is seeking a Registered Nurse Case Manager to join our team in New Jersey. This full-time, Monday–Friday dayshift role focuses on coordinating and delivering compassionate, patient-centered hospice care in patients' homes or approved settings.

You will assess, plan, document, and collaborate with an interdisciplinary team to ensure high-quality, dignified end-of-life care. The RN must have at least 1 year of RN experience in hospice or related settings, be licensed, and

Qualifications

• RN license required to practice as a Hospice Case Manager.
• Minimum of 1 year RN experience in hospice, home health, or related settings.
• Ability to document and communicate clearly with care teams.

Responsibilities

• Perform comprehensive patient assessments and ongoing reassessments.
• Deliver skilled nursing care per Plan of Care in home or equivalent settings.
• Develop and update plans of care with visit frequency and goals.
• Coordinate care with physicians, LPNs, CNAs, social workers, and chaplains.
• Educate patients and caregivers on disease, self-care, medications, and end-of-life needs.
• Report changes in condition to the care team and adjust interventions.
• Maintain timely clinical records and comply with infection control standards.
• Support quality improvement and scheduling initiatives.
