Job summary

Gentiva is seeking a compassionate Registered Nurse - RN Case Manager for a full-time, dayshift role (Mon-Fri) delivering hospice and palliative care in patients' homes or related settings. You'll lead assessments, develop plans of care, and collaborate with an interdisciplinary team to ensure skilled, compassionate care.

This role requires an active RN license in the hiring state, CPR certification, and at least 1 year of RN experience in hospice, home health, or related fields.

Qualifications

• Active RN license in the state of intended employment.
• Minimum 1 year RN experience in hospice, home health, oncology, or related fields.
• Hospice or palliative care experience preferred.
• CPR certification is required.

Responsibilities

• Perform comprehensive patient assessments and reassessments including in-person/telephonic RN assessments and education for patients, families, and caregivers.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care including visit frequency and risk identification.
• Coordinate care across interdisciplinary teams with physicians and nurse practitioners.
• Educate patients and caregivers about disease processes, self-care, end-of-life care, medications and nutrition.
• Document timely clinical records and maintain compliance with infection control standards.
