Job summary

Affinity Health Management seeks a Registered Nurse to plan, organize and direct hospice care in the home and community setting. The role emphasizes community health education and collaboration with families to meet patient needs.

The RN coordinates care, conducts comprehensive assessments, and develops care plans with input from the interdisciplinary team. A current RN license and relevant experience are required to deliver compassionate, quality hospice nursing.

Qualifications

• Graduate of National League of Nursing accredited school of nursing.
• Current licensure in State.
• RN shall have a minimum of one year of professional nursing experience within the last three years; or a BSN from an NLN-accredited program and a current RN license; two years’ experience is preferred, including public health, home care, or hospice nursing.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patient and family to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise plans of care with the interdisciplinary team as patient needs change.
• Coordinate patient care as Case Manager when assigned by Clinical Supervisor.
• Document accurate clinical notes and communicate changes with physicians.
• Participate in on-call duties and ensure equipment and services are available.
