Optimal Hospice in Colorado seeks a Registered Nurse Case Manager to provide comprehensive hospice care for patients and families. You will perform initial and ongoing assessments, develop and implement care plans, and coordinate with the interdisciplinary team to deliver compassionate end-of-life care.

The role requires current RN licensure, NLN-accredited nursing education, and CPR certification. Hospice experience is preferred; strong communication and organizational skills are essential.

Qualifikationen

• Current licensed Registered Nurse in the state where served.
• Experience in hospice or palliative care preferred.
• Strong assessment and care planning skills.
• Excellent communication with patients, families, and team.
• CPR certification maintained.

• Complete initial, comprehensive, and ongoing patient assessments.
• Provide professional nursing care using the nursing process.
• Develop and revise plans of care with patient and family involvement.
• Administer medications and treatments as prescribed.
• Coordinate patient care with the interdisciplinary team.
• Educate patients and families on care and self-care activities.
• Ensure safe environment and continuity of care.
