Bloom Healthcare is seeking a Hospice Registered Nurse Case Manager to coordinate hospice care for patients in the Colorado Springs area. The RNCM will educate families about disease progression, manage symptoms, and lead the interdisciplinary team to meet physical, emotional, and spiritual needs in home or community settings.

• Accredited nursing school graduate.
• Valid CO RN license.
• At least 1 year in med-surg, emergency or critical care nursing.
• Strong clinical assessment capability.
• Excellent communication and interpersonal skills.
• Effective problem-solving skills.
• Detail-oriented with strong time management.
• Computer proficiency.

Responsabilités
